Exhaust manifold bolt missing

I just uploaded a photo to the photo section under 'KC4FWS' folder of what I found yesterday on my coach with the ISC engine. With the over engine hatch (under bed) removed, the photo is taken with an inspection mirror. I was doing some other maintenance and general inspections when I found this. This flange is the attach point for the exhaust manifold to exhaust. Coming from the engine, it is just before the PAC Brake. The flange normally has four threaded studs (looks like 1/4-20 or 5/16-20) with nuts retaining the flange joint.
I discovered that one of studs and nut is missing. Anyone ever see this or hear about it reported before? I am sneding copy to CC and Cummins to see what kind of response I get. The flange seal seems to be in tact with no apparent exhaust leakage at the joint.

Good luck getting a response from any communication with Cummins, the only response I've ever gotten from them is to go to one of their service centers. I've had black smoke from my engine for almost three years now and have spent numerous days, and hours in various Cummins service centers...no fix yet. Of course their out is that all diesels smoke some.....Isn't that one of the problems that the electronic engines was supposed to have fixed. I have pretty much given up on Cummins or CC caring enough about the smoke problem to actually put some engineering heads together to see if they can solve the problem.
And, of course that is their plan, they know that most folks will simple get so frustrated with the process and will eventually go away or their warranty will be up and then it's no longer their problem.
If you will take note they are very interested in you before you buy and they will do the minimum necessary while under warranty but once the warranty is up, even problems you identified prior to warranty expiration is like pulling teeth to actually get anything done. Yes, they will talk nice to you and dance around to entertain you but if it is a tough problem which requires more than the efforts of the wrench bender then I would give them a failing grade.

One problem that numerous CC owners have is the dust storm problem, you would think that a company would value its reputation so highly that they would solve this problem. They keep playing games with it, I just don't understand that. How much could it cost for them to put an engineer onto the problem and figure out a solution and then provide that solution to the owners of the rigs that have the problem?

Yes, CC has a great riding machine but with all the electrical problems and the other identified problems here, it is just a matter of time til the word gets out that their engineering leaves a lot to be desired.

As a retired mechanical engineer, this is the most obvious lack of good engineering.

To design a cooling fan without fully shrouding it is poor. If there is lack of sufficient

discharge area such that the air needs to be blown directly down upon the roadway

it seems to me some more basic engineering is needed.
